Many people worry that tingling hands and feet might mean a stroke and come in for a checkup. But tingling does not necessarily mean a stroke.
Where the problem lies along the sensory pathway — the brain, the spinal cord, or the peripheral nerves — determines the cause of tingling. The distribution of the tingling area helps identify the condition, so it is best to get examined early and find the cause.
However, if tingling suddenly appears on one side of the body together with weakness in an arm or leg, one-sided facial drooping, or slurred speech, these can be signs of a stroke — go to the emergency room without delay.
Causes of Tingling in the Hands and Feet
Most cases of tingling in the hands and feet are related to nerve problems, especially nerve compression — pressure on a nerve. The peripheral nerves spread through the whole body like a net and consist of sensory, motor, and autonomic nerves. When the sensory nerves are affected, the hands and feet tingle, hurt, and feel dull. Sensation travels a long path from the nerve endings in the skin through the spinal cord to the brain, so a problem anywhere along that path can cause abnormal sensation.
The most common cause is nerve compression: a cervical disc or spinal stenosis pressing on nerves in the neck or lower back, or a peripheral nerve compressed in a narrow passage, as in carpal tunnel syndrome. Another major cause is peripheral neuropathy — diabetes is the most representative — in which tingling typically appears symmetrically, starting from the fingertips and toes of both sides.
Vascular disease can also cause tingling. What is loosely called "poor circulation" strictly means abnormalities of the peripheral blood vessels, such as arteriosclerosis or peripheral vasculitis. In arteriosclerosis, deposits build up on the vessel walls, gradually narrowing and blocking the vessels; it can cause not only tingling and numbness but also cerebral infarction and myocardial infarction, so proper management is needed. Rheumatic diseases, thyroid problems, and vitamin deficiency can also cause tingling.
Tingling in the hands and feet is most common in middle-aged and older adults over 40, especially middle-aged women. Heavy physical labor and aging weaken the bones and discs, making nerves easier to compress from external impact or poor posture.
The Korean-Medicine View of Tingling
In Korean medicine, tingling in the hands and feet is classified under bi syndrome. It is understood as a condition in which unhealthy influences such as wind, cold, and dampness invade the body and block the flow of qi and blood. When these influences stay in the skin, sensory symptoms appear; when they invade the bones, muscles, and joints, joint symptoms with pain appear.

Because the treatment differs by cause, an accurate diagnosis comes first. Korean-medicine treatment is based on promoting blood circulation and the flow of qi and blood, combining acupuncture, electroacupuncture, Chuna therapy, and herbal medicine.
Acupuncture is applied to the areas of tingling and pain and to related acupoints to support circulation and help relieve symptoms. Electroacupuncture adds mild electrical stimulation to the needles to enhance circulation and pain relief. Chuna therapy corrects the alignment of the cervical and lumbar spine, which can help reduce pressure on the nerves.
Herbal medicine is prescribed according to the cause: a warming direction when cold is the cause, a heat-clearing direction when heat is the cause, and a direction that removes phlegm-dampness when such impurities are present. When the body is weak, tonifying treatment is combined, distinguishing acute from chronic stages and deficiency from excess. For tingling caused by blood stasis, treatment focuses on dispersing the stasis and improving blood circulation.
Herbal medicine works gradually, but has the advantage of improving the body's overall condition. When a systemic disease such as diabetes or arteriosclerosis is the cause, that condition must be managed together with this treatment.
